Application for Health Certificate for Food Import/Exports

Description

This procedure represents the application process at the Food Safety Bureau (FSB) of the Department of Food and Drugs (FDD) of the Ministry of Health to issue:

Health Certificate for food to be imported into Cambodia
Health Certificate for food to exported to other countries

The Health Certificate for exports can be issued at the request of the exporter, if this is a requirement of the importing country.

Place for submitting application

All Health Certificates need to be applied for and are issued at FSB’s office in Phnom Penh.

Note 1	This letter needs to be drafted by the applicant and can be in free format.
Note 2	Relevant supporting documents for the Health Certificate are: · Business Registration Certificate from MoC · Manufacturing Registration Certificate from MIH · Patent Certificate · Specification of ingredient for final product · GMP, GHP, HACCP Certificate · Certificate of Free Sales (for imports) · 3 packages of samples · Other relevant documents for the product
Note 3	Receipt for fee paid.
Note 4	Health Certificate to allow the import of food into Cambodia is issued or Health Certificate required by importing country for food exports from Cambodia is issued
